Requirements for this form
- Resident Card or Certificate of Citizenship
- Social Security CardMarriage Certificate
- If either of you were previously married must bring the certificate and divorce, date of birth of the ex-spouse and / or the date they were married.
- Places where they lived and worked for the past 5 years
Name, date, place of birth and current residency of both parents (if they passed away in that year died) - If your husband / wife have children, bring the names and date of birth of each
- A picture of you and one of your spouse in passport size
- Check or money order for Immigration in the amount of $535
- The cost of services at DMS is charged separately

- Acta de matrimonio
- Acta de nacimiento de ambos
- Si alguno de los dos fue casados anteriormente deben traer el acta y la sentencia de divorcio, fecha de nacimiento del ex-esposo/a y la fecha en que se casaron.
- Lugares donde han vivido y trabajado durante los últimos 5 años
- Nombre, fecha, lugar de nacimiento y de residencia actual de los padres de ambos ( si fallecieron año en que fallecieron)
- Si su esposo/a tiene hijos, traiga los nombres y fecha de nacimiento de cada uno de ellos
- Una foto suya y otra de su esposo/a tipo pasaporte
Cheque o money order para Inmigración por la cantidad de $535 - El costo del Servicio en la Doctora se cobra por separado
