Requirements for this form

Certificate of citizenship or American Passport
Birth certificate of both of you. If you or your sibling are married you must bring your marriage certificates, if any of you are divorced you need to bring the divorce certificate as well
If your sibling has kids you must bring their names and their date of birth; if they are married – you need to bring name and their date of birth of the spouse
If your parents are or were married you need to bring their marriage certificate
If you are paternal siblings or were declared late – you must bring an affidavit by two or three witnesses (neigbors or friends, they can’t be related to you) that can attest that they know your parent; since when they know them , what they know about the relationship, attest that from that relationship children were procreated (naming you and your siblings) and they always supported you in education, nutrition and financially

Acta de nacimiento de ambosSi usted o su hermano/a son casados, debe traer las actas de matrimonio, si son divorciados el acta de divorcio con la sentencia correspondiente
Si su hermano/a tiene hijos, debe saber el nombre y las fechas de
nacimientos de cada uno; si es casado/a el nombre y la fecha de
nacimiento del esposo/a
Si sus padres estan o estuvieron casados traiga el acta de matrimonio
Si son hermanos de padre o fueron declarado tardio deben traer una declaracion jurada de dos o tres testigos (vecinos o amigos, no pueden ser familia) que digan que conocen a su padre/madre; desde cuando los conocen, que saben de la relacion que hubo entre ellos, que de esa relacion procrearon hijos (nombrandolo a usted y a su hermano/a) y que siempre lo han mantenido tanto en education, alimentacion y en lo economico.
