Requirements for this form

Certificate of citizenship of the applicant’s father / mother
Green Card, Social Security, and all passports starting with that entered the country of their son / daughter
Birth certificate of the child
If you are married to the mother / parent must bring the marriage certificate, and Social security, green card.
If the father is not married to the mother should have custody (custody) of their son / daughter. If the mother lives in the overseas; if within the country or must then go to family court in any other
2 passport photos of the child
Proof of residence:
- Letter from the school
- Letter from the doctor
- Income tax
Check or money order to Immigration of $ 600.00
The cost of the services at DMS is charged separately

Acta de nacimiento de su hijo/a
Si estas casado con la madre/padre debe traer el acta de matrimonio, residencia y seguro Social
Si es el padre y no estás casado con la madre debe tener la patria potestad (custodia) de su hijo/a. Si la madre vive en la Republica Dominicana puede obtener la custodia en el Departamento de Menores y Adolecentes; si estas dentro del país o en cualquier otro debe entonces ir a la corte de familia.
2 fotos tipo pasaporte de su hijo/a
Prueba de que su hijo (a) ha vivido con usted en los Estados Unidos:
- Carta de la escuela
- Carta del doctor
- Impuestos
Cheque o money order para Inmigracion de $600.00
El costo del Servicio en la Doctora se cobra por separado
