Requirements for this form
- In case of loss of the original certificate you must bring a copy. If you don’t have a copy then you may bring an old passport or any other document that contains the green card number
- In case of replacement of the certificate due to incorrect information you must bring the original certificate and all of the documents that you could use to correct the information currently present in the certificate. These documents can include but are not limited to: birth certificate, passport, ID card, marriage certificate, divorce certificate, etc
- 2 passport style photos
- Check or money order for immigration for the amount of $535
- The cost of the service at DMS is charged separately

- En caso de perdida del certificado original debe traer una copia. Si no tiene una copia, puede traer un pasaporte viejo o cualquier otro documento que pueda tener el numero de residencia
- En caso de reemplazo del certificado debe traer el certificado original y todos los documentos que se puedan usar para corregir la informacion actualmente presente en el mismo. Estos documentos pueden incluir pero no estan limitados a: acta de nacimiento, pasaporte, tarjeta de identificacion, acta de divorcio, acta de matrimonio, etc
