Requirements for this form
- Green card o certificate of citizenship
- Social security card
- If you are married and the invitation is going to be made by both you and your spouse then you must also bring the marriage certificate and the green card or certificate of citizenship of your spouse
- Bring the last income tax you filed
- Bring the last six paystubs
- Letter from work
- Letter from the bank (mandatory)
- The cost of service at DMS is charged separately

- Si es casado/a y la invitacion la haran entre los dos deben traer el acta de matrimonio y recidencia o ciudadania de su conyugue
- Traer el ultimo income tax que ha llenado
- Traer los ultimos seis talonarios de pago

- El costo del Servicio en la Doctora se cobra por separado
