Requirements for this form

The green card, certificate of citizenship or American passport
Work letter detailing the following:
- Your Social Security number
- The time you have been working there
- Your position in the company
- Your salary and whether is hourly or not
The last income tax you filed with w-2, if you are an independent contractor you need to call IRS to get a transcript
Your last six pay stubs
Name, date of birth and current address of the person you are petitioning
If the person you are petitioning is older than 18 years old, you must bring a job offer for that person
If you receive Social Security benefits, SSI, or disability benefits, you must bring a car detailing the type of benefit, the amount you receive and since when you started receiving them
If you are not working and do not receive any type of assistance, you must write a letter explaining why you have not filed out your income tax and who has been supporting you during this time
It is necessary to fill out an affidavit for each person who you are petitioning and each son/daughter that will accompany that person
The cost of service at DMS will be charged separately

Si la persona que esta pidiendo es mayor de 18 anos debe traer un oferta de trabajo para esa persona
Si usted recibe Seguro Social, SSI, o disability debe traer una carta detallando el tipo de beneficio, la cantidad que recibe y desde cuando la está recibiendo
Si usted no trabaja y no recibe ningun tipo de asistencia debe hacer una carta explicando porque no ha llenado income tax y quien lo ha mantenido durante todo este tiempo
Es necesario llenar un afidávit por cada persona que esté pidiendo y cada hijo que acompañara a esa persona
El costo del Servicio en la Doctora se cobra por separado
