Requirements for this form
- Certificate of Naturalization, Birth certificate or U.S. Passport of the petitioner and birth certificate of beneficiary
- Social Security card
- Marriage certificate
- If you or your spouse were previously divorced bring the divorce certificate, date of previous marriage and the name and date of birth of previous spouse
- Beneficiary’s passport with the Visa and I-94 ( if you requested the I-94 you must fill out the I-102 form to request a copy before the adjustment)
- Addresses where you and your spouse have lived and worked for the past five years (with dates)
- Name; date and place of birth of each one of the spouses’ parents
- If your spouse has any children bring the names and dates of birth for each one of them.
- Work letter of the petitioner and last income tax filed with W-2
- 2 photos passport style of the petitioner and 4 of the beneficiary
- Check or money order for Immigration of $1,760.00
- Physical exam performed by a doctor authorized by Immigration
- If you were petitioned in the past you must bring the letter of approval
- The cost of the services at DMS is charged separately

- Si usted o su conyugue han sido divorciados deben traer el acta y la sentencia de divorcio, fecha de cuando se casó con su expareja al igual que la fecha de nacimiento
de el/ella. - Pasaporte del beneficiario con la visa y el I-94 (si perdieron el I-94 deben llenar el formulario I-102 para solicitar una copia antes de hacer el ajuste).
- Lugares donde han vivido y trabajado por los últimos 5 años (ambos), nombre de la compañía, dirección y fecha
- Nombre, fecha, lugar de nacimiento y dirección de los padres de ambos; si fallecieron, el ano.
- Si su esposo/a tiene hijos debe traer los nombres y las fechas de nacimiento de cada uno de ellos
- Carta de trabajo del peticionario y ultimo Income Tax con W-2.
- 2 fotos del peticionario y 4 del beneficiado (tipo pasaporte)
- Examen físico hecho por un doctor autorizado por inmigración.
- Si fue pedido anteriormente debe traer la carta de aprobación
- El costo para Inmigración es de $1,760
- El costo del Servicio en la Doctora se cobra por separado
