Requirements for this form

Green card or certificate of Citizenship and Social Security
Act of birth of your son / daughter
If you or your son / daughter are married you should bring the marriage certificate if they have been divorced bring divorce copy
If your son / daughter have children should bring the names and dates of birth of each. If they have a spouse if your son / daughter is married / a bring the information of their spouse
If your child is under 18 years old, you must bring a permission of the other parent giving permission to travel (only if you are not married to the other parent)
If you never married the father / mother of your son / daughter, divorced or declared your child late you need to bring an affidavit of 2 or 3 people (not family) stating the following:
- The type of relationship they have with you (neighbors or childhood friends) and since when do they know you and your child
- Attesting to the knowledge of the relationship you had with the mother / father of the child
- Attest to the financial and educational support your your provided your child with
If your son / daughter is baptized you should bring the baptism certificate

Acta de nacimiento de su hijo/a (inextensa)
Si usted o su hijo/a son casados deben traer el acta de matrimonio. Si han sido divorciados, el acta y la sentencia de divorcio
Si su hijo/a tiene hijos debe traer los nombres y las fechas de nacimiento de cada uno al igual que la del conyugue si su hijo/a es casado/a
Si su hijo es menor de 18 años debe traer una autorización de la madre dando permiso para viajar (solo si no está casado con la madre)
Si usted nunca se casó con el padre/madre de su hijo/a, se divorciaron o declaro a su hijo/a tardío deberá traer una declaración jurada de 2 o 3 personas (no familia) que declaren los siguiente:
- Tipo de relación que tienen con usted (vecinos o amigos de crianza) y desde cuando lo conocen
- Que tienen conocimiento de la relación que usted tuvo con la madre/padre de su hijo/a
- Que saben que usted es el padre/madre y que siempre lo ha mantenido en educación, alimentación y en lo económico
Si su hijo/a esta bautizado debe traer el acta de bautismo
