Requirements for this form

  • Certificate of Citizenship or passport and Social Security
  • Birth certificate of you and your father (mother)
  • If you or your parent is married, you must bring the marriage certificate. If you or your parent have been divorced, the certificate and the divorce decree. If you or your parent is a widower then bring the death certificate
  • If your father / mother has more children you must bring the names and birth dates of each one
  • If you were declared late (after being 2 years of age) by your parent, you must bring an affidavit of 2 or 3 people (not family) stating the following:
    • The type of relationship they have with your parent(s) (neighbors or foster friends) and since when they know each other
    • Attest to the knowledge of your relationship with your parent(s)
    • Attest to the financial and educational support your parent(s) provided you with
  • If you are baptized you must bring the baptism certificate
  • Check or money order for Immigration: $ 535.00
  • The cost of the services at DMS is charged separately

  • Certificado de Ciudadanía o pasaporte y Seguro Social 

  • Acta de nacimiento suya y de su padre/madre (inextensa)

  • Si usted o su padre/madre son casados debe traer el acta de matrimonio.  Si han sido divorciados, el acta y la sentencia de divorcio. Si es viudo/a traer el acta de defunción 

  • Si su padre/madre tiene más hijos debe traer los nombres y las fechas de nacimiento de cada uno

  • Si usted fue declarado/a tardío (después de los 2 años de edad) por su padre/madre deberá traer una declaración jurada de 2 o 3 personas (no familia) que declaren los siguiente:

    • La relación que tienen con su padre/madre (vecinos o amigos de crianza) y desde cuando lo conocen

    • Que tienen conocimiento de la relación de usted con su madre/padre 

    • Que tienen conocimiento de que el padre/madre siempre lo ha mantenido en educación, alimentación y en lo económico

  • Si fue bautizado debe traer el acta de bautismo
  • Cheque o money order para Inmigración: $535.00
  • El costo del Servicio en la Doctora se cobra por separado
