Requirements for this form

Birth certificate of the petitioner and beneficiary
Marriage and divorce (if applicable) certificate with the corresponding act
Valid passport of the beneficiary
Green card, passport, citizenship certificate or drivers license (valid) of the petitioner
Affidavit
The apartment lease for the apartment where you live together, if you do not have a formal lease then the owner of the place has to write a notarized letter to attest that you live there together with a copy of the contact showing the name of the owner.
Letter received in the address shared by both, electricity bill, cable bill, phone bill, shared bank accounts
If you have children in common, bring their birth certificates
If you have a shared life, health or car insurance you should bring them as proof
Pictures of the wedding and of the course of the marriage

Certificado de nacimiento del peticionario y beneficiario
Acta de matrimonio y de divorcio (si aplica) con la sentencia correspondiente
Pasaporte vigente del beneficiario
Tarjeta de Recidencia; Pasaporte, Ciudadania o licencia de conducir (vigente) del peticionario
Affidavit
El contrato de alquiler del apartamento donde viven, si no tiene contrato el dueno debe hacer una carta notarizada diciendo que ambos viven alli junto con la copia del contrato mostrando el nombre del dueno
Cartas recibidas a la misma direccion de ambos; recibos de la luz, cable, telefono, tarjetas de credito, estados de cuentas bancarias a nombre de los dos, etc.
Si tiene hijos en comun, la actas de nacimiento de los mismos
Si tiene seguro de vida, de salud o de vehiculo a nombre de ambos deben tener la pruebas
Fotos de la boda y en el transcurso del matrimonio
