Requirements for this form
- Green Card
- Social Security Card
- Two passport type pictures
- If in the past you have requested a permit, you must bring it with you
- If you are not a green card holder, you must bring your work permit and social security (only people that entered the US with a visa can request a travel permit or advance parole)
- Check or money order for immigration of $660 ($575 for the card and $85 for finger prints). If younger than 11 years old only have to pay $575 for the card.
- The cost at DMS is charged separately

- Cheque o Money Order para Inmigración ($575.00 tarjeta y $85.00 huellas= $660.00 dólares, para mayores de 11 años, menores de 11, $575.00
