Requirements for this form

U.S. Citizen certificarte or U.S. passport and Social Security number
If you or the beneficiary are married you must bring the marriage certificate. If either of you have gotten divorced, you must bring the divorce certificate. You must bring the date of marriage and the date of birth of you ex-spouse. If either of you is a widower, you must bring the death certificate of your spouse.
Birth certificate of both of you
Passport with the via and the I-94 of the beneficiary
If the beneficiary has children, you must bring the names and date of birth of each one of them
You must know the places where the petitioner and the beneficiary has lived and worked in last 5 years (with dates)
4 passport type photos (2×2) of the beneficiary and 2 passport type photos of the petitioner
Money order or check payable to Immigration for $1,760
The cost of the services at DMS is charged separately
