Requirements for this form

Birth Certificate or Certificate of Citizenship and Social Security
Birth certificate of your boyfriend / girlfriend
If either of you has been divorced, you must bring the divorce certificate with the corresponding sentence; date of marriage, as well as the name and date of birth of the ex-spouse
If your boyfriend / girl has children, you must bring the names and dates of birth of each of them
Name, date of birth and place of current residency of each of their parents
Places where you have lived and worked for the last 5 years (with dates)
You and your boyfriend / girlfriend should prepare a sworn statement explaining how, when and where they met (with dates) as well as another statement made by two or three people who know you (can not be family) where they certify that they know of the relationship that exists between you and detail moments that you have shared together
Bring your passport showing the trips you have given to see your boyfriend / girlfriend, and any other type of evidence that shows the relationship between you (photos, receipt of mail and / or phone)
A picture of you and another of your boyfriend / girl passport type
Check or money order for Immigration in the amount of $ 535.00

Acta de nacimiento de su novio/a
Si alguno de los dos ha sido divorciado, debe traer el acta de divorcio con la sentencia correspondiente; fecha en que se caso al igual que nombre y fecha de nacimiento del ex-esposo/a
Si su novio/a tiene hijos debe traer los nombres y las fechas de nacimiento de cada uno de ellos
Nombre, fecha de nacimiento y lugar de la recidencia actual de cada uno de sus padres
Lugares donde han vivido y trabajado por los ultimo 5 anos (con fechas)
Usted y su novio/a deben preparar una declaracion jurada explicando como, cuando y donde se conocieron (con fechas) al igual que otra declaracion hecha por dos o tres personas que los conozcan a ustedes (no pueden ser familia) donde certifiquen que saben de la relacion que hay entre ustedes y detallen momentos que hayan compartido juntos
Traer su pasaporte mostrando los viajes que ha dado para ver a su novio/a, y cualquier otro tipo de evidencia que demuestre la relacion entre ustedes (fotos, recibos de envios y/o de telefono)
Una foto de usted y otra de su novio/a tipo pasaporte
Cheque o money order para Inmigracion por la cantidad de $535
El costo del Servicio en la Doctora se cobra por separado
